AUTHORIZATION TO USE DRIVEWAY

The following authorization is required for the Perry Township Police Department to park its patrol cruisers on the driveway listed above on this form. The person signing this form acknowledges the following: 

I am hereby granting permission to the Perry Township Police Department to park their patrol cruisers in my driveway for traffic patrol purposes, deter unsafe driving and criminal acts.

The undersigned has read and understands the above statements; and, as the owner or on behalf of the owner or lessee of the driveway right-of-way and agrees to the following statement:

I warrant that I have the authority to sign this Authorization; acknowledge and agree that the Perry Township Police Department is not responsible for any damage to pavement, asphalt, dirt/gravel, or subsurface thereof; and hereby release, discharge and waive any and all actions, and causes of actions, and claims for property damage to driveways, sidewalks, and parking areas.

I am the owner of the property on which the driveway is located, and I am the only person who may give the Perry Township Police Department permission to park on the driveway and run traffic operations.
